We support people in our local community to make informed decisions about their end of life care. We do this by supporting your needs and wishes so that you can choose to die in the place and in the way you chose.

Our local end of life care services
We use our effective partnerships with other end of life care services in the community to ensure your coordinated care, giving you, and your family space and time to focus on normal, everyday life. Our highly skilled care workers may even undertake delegated tasks on behalf of district nurses and co-deliver neuro rehabilitation programmes to reduce your need for hospital appointments, minimising any impact to you.
