We stay in contact with hospital staff, review any discharge plans, and speak with you to understand your needs. If helpful, we can visit the ward before discharge to make sure nothing is missed. We can also help arrange any equipment needed at home and ensure our care team is ready to start the moment your loved one returns. We work closely with the hospital to receive a full discharge plan, so we know exactly what support is needed. If mobility needs have changed, we’ll discuss these with both you and your family to make sure your care plan reflects this. We do whatever is needed, whether that’s helping someone in and out of bed, using specialist equipment or installing a falls alarm.

That’s why we maintain direct contact with the hospital discharge team and, if needed, we’ll visit the ward to meet the team and get a clear picture of what’s required.
